First Appointment:
After booking an appointment , you will receive an email from Luminello (our HIPPA compliant electronic medical record [EMR] platform).
You will make an Luminello patient account, enter your payment information, office forms (HIPPA, Privacy, Intake) to complete prior to your visit with us.
We strongly encourage you to read all the forms entirely before signing them. Electronic copies will be available for your reference in your Luminello.
Please make a list of all previous and current medications, doses, including natural supplements used for the appointment. A detailed family history will also be needed.
Please be advised all appointments are via tele health platform only. We strongly advise you to be at home, or at a secured place during your appointment, where privacy can be assured.
For best tele-psychiatry experience possible, please make sure to have a good internet connection and a device with audio-video capabilities.
